Informed Diagnostics & Comprehensive Treatment
When partnering with a facility, we work closely with residents to develop treatment plans tailored to their unique needs and goals.
Evaluation & Assessment
As we begin working with older adult residents, we conduct a thorough evaluation to assess their mental, physical, and nutritional health. This enables our clinical team to create a detailed list of the resident's challenges, strengths, and weaknesses, helping us determine the appropriate level of care based on their physical, emotional, and cognitive needs.
Therapeutic Treatment
Following the assessment, our clinical team crafts personalized treatment plans targeting specific areas for growth. Utilizing a blend of therapy modalities—including individual, group, and family therapy—we aim to equip residents with effective coping strategies to mitigate the recurrence of their disorders and prevent relapse. Our approach emphasizes teaching exercises and skills that residents can use in their daily lives, leading to improved self-esteem and a greater sense of achievement.
Aftercare & Review
Our clinical team collaborates closely with facility staff to provide ongoing education and training, ensuring continuous care and establishing effective aftercare planning for residents. We perform regular, systematic reviews of our program's effectiveness by checking in with residents, families, and staff to monitor progress and improvements. Our primary objective is to enhance the quality of life for residents while increasing satisfaction among family members.
